  1. Middletown Office: Located at the rear of the Middletown Commons Plaza.
  2. Drive down the left side of the plaza building, take a right, and the office is the first business on the right with a green awning.
  3. Fall River Office: Located in Suite 105 on the first floor of the building at 1822 North Main Street, which is the same building as the Fall River branch of The Massachusetts Registry of Motor Vehicles.
