  1. The Azalea Orthopedics Main Office in Tyler is split into three sections: Main Level entrance (left door), Joint Reconstruction Center entrance (right door) adjacent to the Main Level, and the Spine Center entrance on the lower level facing Paluxy Drive.
  2. The Palestine clinic is in the same building as Baylor Scott & White Texas Spine & Joint Urgent Care Clinic.
  3. The Nacogdoches office is north of SFA University on University Dr. The South Broadway clinic is in the same building as Baylor Scott & White Texas Spine and Joint Urgent Care Clinic, with its entrance on the Northside of the building.
