  2. For online booking with your primary care provider, you may book a 15-minute in-clinic appointment for new concerns or follow-up visits. Physicals, well-baby visits, or procedures are not available for online booking.
  3. For urgent issues in the next 2-3 days, appointments longer than 15 minutes, if you do not have an OHIP number, or need an appointment with an allied health provider, please call the clinic.

  1. Bathurst site is located at the southwest corner of Bathurst Street and College Street.
  2. Garrison Creek is located on the north side of St. Clair Avenue West, east of Oakwood Avenue at Robina Avenue and St. Clair Avenue WeSt.

Bathurst site serves communities from Bloor Street to Lake Ontario; Dufferin Street to Yonge Street. Garrison Creek site serves communities from Eglinton Avenue West to Bloor Street; Keele Street to Spadina Avenue.
