Instructions

  1. Please print the New Patient Questionnaire/History Form, Consent to Treat and Notice of Privacy Practices, and Demographics and Contact Authorization forms and bring them completed to your appointment.
  2. A mask that covers the nose and mouth is required for everyone.
  3. Temperature readings will be taken at the front desk.

Getting Here

  1. Due to construction that will last into 2022, the primary road to our Omaha office is closed.
  2. You will no longer be able to use 168th street between Pacific and Center.
  3. Please review alternate routes.
