Instructions

  1. You will need your health card for your initial meeting. If you do not have one, you will need to type in "0"s when prompted for your health card number. Then, let one of the providers know that you don't have one.
  2. The initial intake appointment can take up to 1-2 hours depending on provider availability, etc. Subsequent appointments will be shorter; however, the length of time depends on how many providers you are seeing.
  3. If at any time you are having technical issues or need support, you can call and leave a message at 519-758-0008. We will get back to you as soon as we can to help.
