Instructions
- If planning to take a loved one home (out-on-pass), contact their social worker to make proper arrangements.
- Car Transfer Training must be scheduled and completed with the Physical Therapy Department prior to leaving for out-on-pass.
- Nursing Care / Medication Review with resident's designated representative is required prior to leaving for out-on-pass.
