Cherokee Nation Health Services operates 11 healthcare facilities, including a hospital and nine health centers, providing a wide range of inpatient and outpatient services. These include primary care, dental, behavioral health, and various specialty services, serving over 100,000 patients.

Services are provided to all registered members of federally-recognized tribes, and mothers carrying future tribal citizens.

Instructions

  1. To ensure that CNHS has all relevant information, patients will be asked to complete registration forms, present insurance forms and eligibility documents, and sign other required paperwork during their first visit after October 1st.
  2. Prior to October 1st, patients can complete a medical records release form to ensure their records are transferred from CIH to CNHS. Submitting this form will also allow CNHS to create a patient chart within our system. Forms can be completed with the Registration team onsite at Claremore or at any CNHS location.
  3. Clients for Jack Brown Center must be citizens of a federally recognized tribe.
