Instructions

  1. Complete health questionnaires powered by Instant Medical History no more than 7 days prior to your scheduled appointment.
  2. For your eye exam visit, complete the EYE CLINIC questionnaire which includes Fall Risk questions.
  3. For your Medicare Preventive Visit, complete the MEDICARE PREV. VISIT questionnaire which includes Fall Risk questions.
