Instructions

  1. All patients must have a physician referral and schedule an appointment in advance.
  2. All orders should include: Patient name and date of birth, Diagnosis (including ICD-10 code), Physician orders, Physician's signature (including date and time).
  3. Out-of-area patients must have orders signed by the physician.
