Instructions

  1. Bring a copy of any X-rays or other diagnostic studies, such as MRI and CT scans, to your first appointment.
  2. Print and complete the history form for the physician you will be seeing, as well as the demographic sheet, and patient financial policy, and bring them to your appointment.
  3. Arrive 15 minutes prior to your appointment to complete new patient registration.
