Best Fit Patients

Individuals aged 18 or older with severe and/or persistent mental illness, often affecting daily functioning, who have difficulty maintaining treatment or lack personal support systems.

Instructions

  1. Individuals must be willing to participate in the community or be clients legally aligned to services through such programs as Community Treatment Orders.
  2. Clients must be triaged by the Assertive Outreach nurse prior to acceptance for services.
