Individuals aged 18 or older with severe and/or persistent mental illness, often affecting daily functioning, who have difficulty maintaining treatment or lack personal support systems.
Instructions
- Individuals must be willing to participate in the community or be clients legally aligned to services through such programs as Community Treatment Orders.
- Clients must be triaged by the Assertive Outreach nurse prior to acceptance for services.
