Instructions

  1. All patients will need a referral faxed to our office from a physician with records related to what they are going to be consulted for.
  2. If you have been seen by another urologist we will need all your records regarding your previous urologic treatment, hospitalizations, x-rays, and/or lab results.
  3. Complete new patient forms and return them to our office at least two days prior to the appointment or we will have to reschedule.

Getting Here

  1. From Interstate 5: Take exit number 678 for Highway 44 WeSt. Turn LEFT on Court St. Drive south on Court St. until the street splits like a Y.
  2. Stay to the RIGHT, and follow the blue “H” hospital sign.
  3. Turn RIGHT on Rosaline Ave. At the top of the hill you will come to a four way stop go straight, the street changes from Rosaline to Edith Ave. Take the first RIGHT into Clairmont Doctors Park.
