Instructions
- All patients will need a referral faxed to our office from a physician with records related to what they are going to be consulted for.
- If you have been seen by another urologist we will need all your records regarding your previous urologic treatment, hospitalizations, x-rays, and/or lab results.
- Complete new patient forms and return them to our office at least two days prior to the appointment or we will have to reschedule.
Getting Here
- From Interstate 5: Take exit number 678 for Highway 44 WeSt. Turn LEFT on Court St. Drive south on Court St. until the street splits like a Y.
- Stay to the RIGHT, and follow the blue “H” hospital sign.
- Turn RIGHT on Rosaline Ave. At the top of the hill you will come to a four way stop go straight, the street changes from Rosaline to Edith Ave. Take the first RIGHT into Clairmont Doctors Park.
