- If Sun River Health has not already assigned you to a Payment Group, or if your household income has recently changed, you must bring at least one of the following documents with you to your Health Center visit: Most recently filed income tax return, Paychecks stubs from current or most recent job, Most recent W-2.
- If you are unable to bring one of these documents, we will help you fill out a one year self-attestation form and see which payment group you qualify for.
