Patient Instructions
  1. 1A valid health card is required or fees will apply (CASH ONLY)
  2. 2Narcotics will not be prescribed
  3. 3Medical masks are recommended at both locations
  4. 4Completion of forms is at the discretion of the attending physician
  5. 5Wart treatments not available at this time
  6. 6Check-in with the receptionist upon arrival
  7. 7If you do not have a valid health card, cash payment for the visit will be required (minimum $70). You must provide a valid health card within 15 business days for a refund.
  8. 8To avoid delays for telephone assessments, have your health card, temperature, blood pressure (if applicable), list of allergies, child's weight, and current medications ready. Be available to receive a call back (phone number may appear as “blocked” or “no caller ID”) and be in the province of Ontario.
