Instructions

  1. Patients will be asked to complete registration forms, present insurance forms and eligibility documents, and sign other required paperwork during their first visit after October 1st.
  2. Prior to October 1st, patients can complete a medical records release form to ensure their records are transferred from CIH to CNHS.
