Instructions

  1. Patients should not relay prescription refill requests and other personal treatment information via online inquiry, as the information will be delayed in getting to the provider. For these issues, patients should call the telephone number of the program where they are seen, or call (251) 450-2211, or toll-free (866) 547-4845.
  2. For new patient intakes for substance use services, call (251) 450-2211 for information.
  3. Required information for substance use services includes proof of income (most recent 1040 income tax return with W-2(s) and 1099, most recent two (2) pay stubs, earnings record from ssi.gov, proof of Social Security income, verification letter if receiving food stamps, proof of alimony, child support, unemployment, pension, etc.) or a notarized letter explaining support from relatives/friends if no income, or benefits verification letter from the Social Security office for proof of no income.
