- Complete and submit the Informed Consent for Teletherapy/Telehealth form before your visit.
- For ADHD treatment, provide documentation of ADHD diagnostic evaluation and treatment from a previous physician or licensed professional (Neuropsychological assessment or psychoeducational assessment).
