Patients seeking medical, surgical, or cosmetic dermatologic care, including pediatric dermatology and Mohs micrographic surgery.

Instructions

  1. A Card on File (COF) policy will be implemented starting July 1, 2025, for scheduling appointments. This card will be charged for no-shows, cancellations less than 24 hours in advance, and unpaid balances after 30 days of a statement being sent.
  2. For cosmetic appointments, a $50 fee will be charged for cancellations less than 24 hours or no-shows. A $50 deposit is required for Sclerotherapy/Dermal Filler/Physician Laser appointments, which is non-refundable for cancellations less than 24 hours or no-shows.
  3. Patients with a remaining deductible of $250.00 or more will be required to pay a $100.00 deposit on the day of their visit.
