Instructions

  1. Please call the office to schedule your MDVIP Annual Wellness Physical (AWP) 1 year after your last Annual Physical or Medicare Annual Wellness Visit.
  2. Allow 1 hour for the pre-physical testing visit, and then an additional 1.5 hours for the Annual Wellness Physical visit with your physician.
  3. Fast for 8 hours prior to coming into the office for pre-physical testing, unless you have a medical condition that makes fasting unsafe. Water, black coffee, and tea are fine.
