  1. Please call (604) 883-2764 at 8am on Tuesdays and Thursdays to book physician appointments.
  2. For PHHC Nursing Lab & Clinic, patients can access with a requisition/referral from a primary care provider; call the Health Centre to schedule an appointment.
  3. For Chronic Disease Management (CDM) Clinic, you can access either through a referral from a primary care provider or by self-referring; call the Health Centre reception at 604-883-2764.
