Instructions

  1. Bring a completed Health History Form, including a list of all prescribed and over-the-counter medications, vitamins, herbs, and supplements with doses.
  2. Bring an Electrocardiogram (if done within the last two years).
  3. Bring laboratory results from within a year, including Comprehensive Metabolic Profile, Lipid Profile, Hemoglobin A1C, Ultra-Sensitive or High-Sensitivity C-Reactive Protein, CBC w/Diff., Thyroid Profile (TSH, free T4), Insulin Level, Vitamin B12, and Vitamin D, 25-OH.
