  2. When you come for your appointments, please remember to bring the following Driver's License or State ID Referral if required by your insurer, Insurance information Copies of operation records, medical records, x-rays, MRIs, CT scans and so on from prior doctor visits.
  3. To cancel an appointment: Cancellation policy please call 24 hours prior for your physical appointment or $25 charge will be billed.
