Instructions

  1. If you have any respiratory symptoms, sore throat, feel unwell or have a fever: PLEASE WEAR A MASK AT ALL TIMES WHILE IN THE OFFICE (INCLUDING IF YOU ARE IN THE ROOM BY YOURSELF).
  2. Please request that your pharmacy fax us for any refills and we will be happy to do it by fax (try to do it at least 2 weeks in advance).
  3. All controlled substances: narcotics, benzodiazepines and ADHD medications have to be refilled in person.
