Instructions

  1. Please try to arrive 30 minutes before clinic close at break and/or end of day for vaccination services.
  2. Bring all vaccination records and a piece of identification for the child/youth receiving the vaccine(s) or their Ontario Health Insurance Plan (OHIP) card if available. An OHIP card is not required if you do not have one.

Getting Here

  1. The ACB (African, Caribbean and Black) Wellness Resource Centre hub at 610 Donald St is not physically accessible to those with mobility disabilities.
  2. The closest accessible Hub is at Rideau High School (Rideau-Rockcliffe Community Health Centre), located at 815 St. Laurent Boulevard (Main Entrance).
  3. For AMA Community Centre, use the side entrance, not the mosque main door.

Available — Phone consultations are available with a public health nurse for vaccination questions, Monday to Friday from 9:00 am to noon and 1:00 pm to 4:00 pm. Health811 offers free, secure, and confidential health advice via phone, available in English and French with translation support.
