  1. For patients scheduling who are under 18 years of age (19 in Alabama) please make sure you have permission from your parent or legal guardian to schedule this appointment. Your parent or legal guardian must accompany you on your initial visit and on certain subsequent visits to provide appropriate informed consent.
  2. If you have a VA or Tribal referral, please call 715-524-5994 to schedule your appointment.
  3. Please call 715-524-5994 to schedule the following appointments: Cosmetic/Laser treatments, Surgery or removals, Accutane.

Getting Here

  1. The office is easy to find at 100 County Road B, Shawano, WI 54166, inside the Theda Care Family Medicine building of Theda Care Hospital.
  2. The entrance is “#1” to the right of the main hospital entrance.
  3. Effective November 6, 2024, the office will be relocating to a new space just one mile south at N4531 State Highway 22, Suite 5, across from Arby’s and the Shell station.
