- Patients/Parents are responsible for accurate and up to date health card information. Please bring your health card for each visit.
- Failure to provide accurate and up to date health card information will result in your claim to OHIP being rejected. Rejected claims are subject to $20.00 administrative fees plus the cost of visit.
- Patients will be charged $50 for a no-show appointment. Please be considerate and attend your appointment or cancel at least 12 hours in advance so that other patients can access the doctor and avoid any no-show penalties.
