Patients of all ages with orthopedic injuries to the hip, knee, foot and ankle, elbow, shoulder, neck and spine, and those needing pain management.

Instructions

  1. Arrive 15 to 20 minutes before your appointment to complete clinic and insurance forms.
  2. Bring your photo ID or military ID.
  3. Bring a letter of authorization from your insurance carrier if your treatment is covered by Workers Compensation.

Getting Here

  1. The Main Level (left entrance) is located on the second floor, adjacent to the Joint Reconstruction Center (right entrance).
  2. The Spine Center is located on the lower level of the building and is the only entrance that faces Paluxy Drive.
  3. You cannot access the upper levels from the Spine Center or vice versa.
