Instructions

  1. Students are reminded to submit proof of immunizations against rubeola, rubella, mumps, meningitis (strains A,C,W,Y, and B), diphtheria, and tetanus to the PUSH immunization portal.
  2. If you do not submit proof of immunization, a hold will be placed on your account and you will be unable to register for future classes.
  3. Immunization records must include the student’s full name, be in English, and legible.
