Instructions

  1. For yearly physicals with Dr. Sherman on Thursdays, fast (no food or drink for 12 hours prior, except water for medication) if diabetic or taking cholesterol medication.
  2. Bring completed New Patient Questionnaire for first annual appointment.
  3. Bring completed Annual Questionnaire for renewal of membership.
